Telehealth Informed Consent

This Telehealth Informed Consent applies to care provided by J&M Entrepreneurs, PLLC, doing business as The Hormone Experts (“The Hormone Experts,” “we,” “us,” or “our”). Please read it before you begin treatment. You may ask questions at any point in your visit.

1. What telehealth means

Telehealth is the delivery of healthcare using secure video, audio, messaging, and electronic records instead of an in-person office visit. Your provider reviews your intake, symptoms, and lab results, meets with you remotely, and, when clinically appropriate, issues a treatment plan and prescription.

2. When a provider-patient relationship begins

Browsing this site, completing the eligibility quiz, or submitting the intake form does not create a provider-patient relationship. That relationship begins only after you complete intake, are reviewed by a licensed provider, and participate in a live consultation with that provider.

3. Where we provide care

We support patients across the United States. Availability of specific services varies by state based on provider licensure and applicable state law. You must be at least 18 years old and physically located in a state where your provider is licensed at the time of your visit.

4. Benefits and limitations

Telehealth can improve access, reduce travel, and shorten wait times. It also has limits. Your provider cannot perform a hands-on physical examination, and technology can fail or delay a visit. In some cases your provider may determine that telehealth is not appropriate for you and may refer you for in-person evaluation, imaging, or emergency care.

5. Prescribing, labs, and monitoring

Treatment is prescribed only when a licensed provider determines it is medically appropriate for you. Not everyone who applies qualifies, and submitting an application does not guarantee a prescription. Lab testing is ordered when medically necessary and is interpreted by our providers. Ongoing lab work and follow-up may be required as a condition of continuing therapy, including metabolic markers, thyroid function, and other relevant values.

Some therapies, including certain peptides and compounded formulations, may be prescribed off-label. Your provider will review the expected benefits, risks, and alternatives with you before you begin.

6. Risks and side effects

As with any medical therapy, risks exist. GLP-1 and metabolic therapies may cause nausea, vomiting, diarrhea, constipation, injection-site reactions, gallbladder problems, pancreatitis, low blood sugar (particularly when combined with other glucose-lowering medications), and other effects. Hormone therapies carry their own risks, which your provider will review with you. Tell your provider about every medication, supplement, and medical condition you have, and report new or worsening symptoms promptly.

Results vary from person to person. No specific amount of weight loss, symptom relief, or lab improvement is promised or guaranteed.

7. Compounded medications

When your plan includes a compounded medication, it is prepared for you by a licensed compounding or outsourcing pharmacy. Compounded medications are not reviewed or approved by the FDA for safety or effectiveness, even when they contain the same active ingredient as an FDA-approved product. Your provider will explain why a compounded formulation is being used in your case.

8. Privacy and recordkeeping

Your visits and records are maintained in HIPAA-secure systems that use encryption and access controls, and are accessed only by personnel involved in your care or in operating the practice. Our Privacy Policy describes how information is collected and shared. No telehealth platform can guarantee absolute security.

9. Emergencies

Telehealth is not for emergencies. If you are experiencing chest pain, difficulty breathing, severe abdominal pain, a severe allergic reaction, thoughts of harming yourself, or any other emergency, call 911 or go to the nearest emergency department.

10. Your rights

You may ask questions, request an in-person referral, decline any recommended treatment, and withdraw this consent at any time by contacting us. Withdrawing consent stops future telehealth care and does not change care already provided. Cancellation of billing is handled under our Refund & Auto-Bill Policy.

11. Consent

By completing intake and proceeding with a visit, you acknowledge that you have read and understood this consent, that you have had the chance to ask questions, and that you consent to receive care by telehealth from The Hormone Experts.
